Personal data protection

In accordance with Law No. 29733, the Personal Data Protection Law and its Regulations, I declare that I have been informed by the University of Piura [THE UNIVERSITY] with RUC No. 20172627421, and I authorize that my personal data, recorded in this form, as well as those obtained by virtue of the relationship with THE UNIVERSITY, including image, voice, signature and electronic signature, be stored in the databases of THE UNIVERSITY [Database of the Customer Management Office - Piura Campus, with code: RNPDP-PJP No. 837, Database of Commercial Management (Admissions - Customer Management) - Lima Campus (RNPDP-PJP No. 847) or Marketing Database of the PAD (RNPDP-PJP No. 2252)] or others that comply with the security measures required by the regulations on the matter; and are processed by THE UNIVERSITY and/or by natural and/or legal persons to whom it delegates or entrusts; who may process said data for the following purposes: (i) For contact through the telephone or cell phone number, home address or email provided to UDEP and/or recorded in the form, for commercial, marketing and/or sending information about the University's products and services, physically, virtually or remotely, including the use of social networks, calls, call centers, telephone calling systems and sending text messages to cell phones or electronic messages, (ii) For invitations and management of participation in events and activities organized by UDEP, (iii) For capturing your image and/or voice in photographs and/or videos at the events and/or activities in which you participate, as well as their publication and/or dissemination in physical or virtual media, including social networks, (iv) to manage application and admission processes, (v) To maintain internal security, (vi) To comply with the requirements of consumer protection regulations and comply with legal or regulatory requirements at national and international level, (vii) for statistical, historical and research purposes.

During events, workshops, talks, or any type of academic, associative, cultural, recreational, or sports activity, whether virtual or in-person, THE UNIVERSITY may take photographs, record videos, or broadcast live for publication and/or dissemination on social media or other physical or digital media owned by UDEP or contracted by UDEP for this purpose, or on social media. Your attendance and/or participation in these activities implies acceptance of the publication and/or dissemination of your image and/or voice.
I hereby acknowledge that I have been informed that failure to provide the information provided on this form will prevent me from being contacted by the University or participating in events organized by it.

THE UNIVERSITY guarantees the possibility of exercising the rights of access, rectification, cancellation and opposition, as well as revoking your consent, when appropriate, by requesting it in writing to the following addresses: Calle Mártir José Olaya 162, Miraflores, Lima, Av. Ramón Mugica 131, Urb. San Eduardo, Piura or Jirón Aldebarán 160 Surco, Lima; or to the email address: datospersonalesudep@udep.edu.pe. Likewise, a claim may be filed with the National Authority for the Protection of Personal Data at the Reception Desk of the Ministry of Justice and Human Rights: Calle Scipión Llona 350, Miraflores, Lima, Peru, if you consider that the exercise of your rights has not been adequately addressed.

Personal data will be stored and/or processed as long as necessary to fulfill the stated purposes for a minimum period of two (02) years and a maximum of ten (10) years, or until you exercise your right of revocation. Consequently, I hereby authorize THE UNIVERSITY to carry out the aforementioned activities.
