PERSONAL DATA PROTECTION

In accordance with the provisions of Law No. 29733 and its Regulations, I declare that I have been informed and authorize that the personal and/or sensitive data contained in this form, as well as those obtained by virtue of my relationship with the University of Piura, (hereinafter THE UNIVERSITY), including my image, voice, signature, electronic signature and/or fingerprints, be incorporated and stored in THE UNIVERSITY's Database, or others that comply with the security measures required by the regulations on the matter; and be processed by THE UNIVERSITY and/or by natural and/or legal persons to whom it delegates or entrusts. In this regard, THE UNIVERSITY and/or the natural and/or legal persons to whom it delegates or entrusts such processing may process said data for the following purposes: (i) for the execution of the relationship with the data subject, (ii) for the financial, accounting, and collection management of payments made by participants, (iii) to establish and/or maintain contact and to inform and/or promote THE UNIVERSITY's activities, (iv) for statistical and/or research purposes, (v) for commercial prospecting, marketing, and/or sending advertising and information about products and services, (vi) to be shared with the UDEP-Alumni community and any other academic or administrative unit of THE UNIVERSITY. The data will be processed as long as deemed necessary for the stated purposes. The University hereby acknowledges the possibility of exercising the rights of access, rectification, cancellation, and opposition, and any other rights provided for in accordance with the applicable regulations, by submitting a written request to the following addresses: Av. Ramón Mugica 131, Urb. San Eduardo, Piura or Calle Mártir José Olaya 162, Miraflores, Lima. Therefore, I hereby authorize the University of Piura to carry out the aforementioned activities.

The data will be processed as long as they are considered necessary for the purposes indicated. THE UNIVERSITY records the possibility of exercising the rights of access, rectification, cancellation and opposition, and any other provided, in accordance with the regulations on the matter, requesting it in writing at the following addresses: Av. Ramon Mugica 131, Urb. San Eduardo, Piura or Calle Martir José Olaya 162, Miraflores, Lima. Consequently, I hereby authorize the University of Piura to carry out the aforementioned activities.
